The 20-year-old landed awkwardly after winning a header in the dying stages of Sunday's 2-1 defeat at Old Trafford and was forced to withdraw from the England squad after Martinez confirmed that the youngster had suffered ligament damage.
The full extent of the problem has now been ascertained and the former Barnsley man will face an extended spell on the sidelines, not featuring again for the Toffees until 2015.
"John will need surgery, which will give us a perfect recovery for his left ankle," Martinez told the club's official website.
"We expect him to be out for probably the next 10-to-14 weeks, depending on how everything goes.
